Online Application!
To be considered for admission into the
program, you must:
- Complete all items on the Application
Form, the Confidential Financial Statement,
and the Supplemental Information Form.
These forms are now available online by
clicking here.
- Provide documented evidence of income
information. Provide a signed
copy of your most current Federal Income Tax
Forms. If you are an
independent student and do not have taxable
income and do not file federal tax forms,
you may request a Financial Confirmation
Form from our office.
- Provide medical and/or psychological
documentation if you have indicated a
disability. Learning Disability
documentation must be no more than three
years old.
After we receive this information, you will
be contacted to set up an Application Interview
with our Director. This meeting will be
used to determine your eligibility for our
program, tell you more about the program and
find out how we can best help you.
