Selective Service Registration

The Financial Aid Office at Purdue University North Central will request this documentation only if it is needed to complete your financial aid file.

Instructions:

1. Submit one of the following to the Financial Aid Office if you have already registered with Selective Service:
   • A copy of your selective service registration card
   • A print out of the selective service registration confirmation page
2. Provide the Member 4 copy of your DD214 (“Certificate of Release or Discharge from Active Duty”)
3. Register for Selective Service and then print out and submit your confirmation page to the Financial Aid Office if you have not registered and are required to register.

Important: Remember to include your PUID and name on all documentation submitted to the Financial Aid Office.
