Postponement Request for Financial Aid Students
(Please read before signing below)

I will be attending Purdue University North Central during the 8-week Summer Session. Please postpone the due date of my class fees. I have applied for financial aid and/or a student loan. If the financial aid does not cover my total charges due this semester, I have enclosed payment for the balance due. I agree to make full payment within one week of the notification date if I am not eligible for the aid for which I have applied, or if I have not completed the necessary paperwork to receive the type of aid applied for. I realize that it is my responsibility to make sure that all of the necessary paperwork has been completed and turned into the Financial Aid Office prior to May 23, 2003. I also realize that a Student Loan application will not be processed until a FASFA application has been filed. I am aware that it is my responsibility to cancel my classes through the Registration Office if I choose not to attend, and if this is done prior to the end of the first week of class, no class fees will be due. I also realize that if I drop after the first week of class, I will be responsible for the balance due after the refund has been applied. I understand that my records will be encumbered, and a $5.00 encumbrance fee will be charged. I also understand that I am responsible for the balance due and any attorney fees and other costs and charges necessary for the collection of any amount not paid when due.

I have checked the types of financial aid that I will be receiving for the 8-week Summer Session 2003 and have entered the dollar amounts next to the aid that I will receive (enter aid not listed on blank lines):

If an agency or employer is paying my tuition, I am enclosing an authorization from them. If I am receiving a scholarship, I have enclosed a copy of the letter and/or award certificate.

If my financial aid is not enough to cover all of my 8-week Summer Session 2003 charges, I have enclosed payment for the balance. I understand that my fees will not be postponed if the balance is not paid by the due date.
