Financial Aid - Forms

Parent Income Follow-up Form

The Financial Aid Office at Purdue University North Central will request this documentation only if it is needed to complete your financial aid file.

On your 2012-2013 Free Application for Federal Student Aid (FAFSA) your parent(s) reported minimal or no income, assets or savings for calendar year 2011. Additional information is being requested to clarify your parent’(s) income. Further processing of your financial aid will not occur until the information requested below has been received by the Financial Aid Office.

Instructions:

  1. Print the Parent Income Follow-up Form.
  2. Submit the completed form to the Financial Aid Office.

Important: Remember to include your PUID and name on all documentation submitted to the Financial Aid Office at Purdue University North Central.
