Verification Worksheet (Dependent)

Your Free Application for Federal Student Aid (FAFSA) was selected for a “Verification” process which compares information from your FAFSA with your and your parent(s)’ 2012 Federal income tax information and/or other  financial documents. Based on this review Purdue University North Central may need to make electronic corrections to your FAFSA information.

Your application will be placed on hold until the requested documentation is provided and appropriately completed. Please submit the requested documents to the Financial Aid Office as soon as possible. Failure to comply may result in a cancellation of some or all of your aid.

Instructions:

• Print the Dependent Verification Worksheet for 2012-2013
• Submit the completed worksheet signed by you and a parent along with any additional documentation requested to the Financial Aid Office at Purdue University North Central
