Child Support Follow-up (Parent)

The Financial Aid Office at Purdue University North Central will request this documentation only if it is needed to complete your financial aid file.

Verification is a process required by the U.S. Department of Education to review the accuracy of information reported on the Free Application for Federal Student Aid (FAFSA). A discrepancy exists between the amount of child support received which your parent(s) reported on the FAFSA and the amount reported on your Verification Worksheet. Further processing of your financial aid will not occur until the information requested below has been received by the Financial Aid Office.

Instructions:

1. Print the Child Support Follow-up (Parent) Form.
2. Submit the completed form to the Financial Aid Office.

Important: Remember to include your PUID and name on all documentation submitted to the Financial Aid Office at Purdue University North Central.
