Financial Aid - Forms
Citizenship Documentation
The Financial Aid Office at Purdue University North Central will request this documentation only if it is needed to complete your financial aid file.
Instructions:
Submit a copy of one of the following acceptable forms of documentation of U.S. Citizenship to the Financial Aid Office:
- Your (the student's) birth certificate showing that you were born in the United States which includes Puerto Rico, Guam, the U.S. Virgin Islands, American Samoa, Swain's Island or the Northern Mariana Islands
- Form FS-240 ("Consular Report of Birth Abroad")
- Form FS-545 ("Certificate of Birth-Foreign Service")
- Form DS-1350 ("Certification of Report of Birth")
- Form G-639 (the Freedom of Information Act Form)
- A United States passport, current or expired, except limited passports. (In the case of nationals who are not citizens, it will be stamped "Non-citizen National".)
- Form N-560 or N-561 (“Certificate of Citizenship”), which contains at least the following information:
- Student's name
- Certificate number (found in the upper right hand corner)
- Date of issuance of the certificate
- Form N-550 or N-570 (“Certificate of Naturalization”), which contains at least the following information:
- Student's name
- Certificate number (found in the upper right hand corner)
- Alien Registration Number (A-number)
- Name of the court where the naturalization occurred
- Date of naturalization
Note:
- U.S. Citizenship and Immigration Services (USCIS) has confirmed that any of the above documents may be photocopied as long as it is done for lawful purposes.
- Social Security cards and driver's licenses are not acceptable forms of documentation of citizenship.
Important: Remember to include your PUID and name on all documentation submitted to the Financial Aid Office at Purdue University North Central.
