Limited Criminal History Report
A limited criminal history check is a required prerequisite for all students enrolled in courses requiring the student to enter an elementary classroom for observation/participation. Students may not enter the elementary schools and may be administratively dropped from the course if they do not have a criminal record report. Criminal history reports need to be updated yearly. An original limited criminal history check is required for licensure.

Request for Limited Criminal History Report (Paper form to be sent to Indianapolis)
Complete the personal information at the top of the Request for Limited Criminal History Information Form 8053. (YOU are the "Requesting Agency" and the "Subject of Request") Check (2) for license, check (10) for all others. (No fingerprinting is required as of July 1, 1998.) Print and sign at the bottom.
Submit completed Form 8053 and a cashier's check or money order for $7.00 payable to:

Results will be returned to you in three to four weeks. The limited criminal history report is valid for a period of one year from the date of issuance regardless of the number of license actions requested within that time frame.
