Online Application!

To be considered for admission into the program, you must:

1. Complete all items on the Application Form.
2. Provide documented evidence of income information. 
   Provide a signed copy of your most current Federal Income Tax Forms.  If you are an independent student and do not have taxable income and do not file federal tax forms, you may request a Financial Confirmation Form from our office
3. The Disability Services Coordinator will verify eligibility of a disability. 

Once the information is received by Student Support Services, you will be contacted to set up an Application Interview with the Director. This meeting will be used to determine your eligibility for our program, discuss program services and requirements and find out how we can best help you be successful at PNC.
