PROCEDURES FOR PROVISION OF ACCOMMODATIONS FOR STUDENTS WITH DISABILITIES
Supersedes Administrative Memorandum 7-2004, November 17, 2004; Administrative Memorandum 4-99, August 16, 1999; and Administrative Memorandum No. 3-2002, July 26, 2002.
It is the policy of Purdue University to ensure that each qualified student with a disability may pursue his or her educational program free from discrimination based on a disability. Purdue University will not deny admission to or enrollment in educational programs and courses on the basis of disability. The University is committed to taking the necessary action to make academic programs, nonacademic and extra-curricular services and activities accessible to qualified individuals with disabilities.
Equal Opportunity Memorandum No. 3, Nondiscrimination on the Basis of Handicap in Educational Programs and Activities, issued October 5, 1977, outlines in detail the policy of the University. It reads, in part:
III. Treatment of Students
A. Academic Adjustments
The University shall make modifications to its academic requirements as are necessary to ensure that such requirements do not discriminate on the basis of disability against a qualified student. Academic requirements which are essential to the program of instruction or relate directly to a licensing requirement shall not be considered discriminatory. Modification may include substitution of specific courses, adaptation of the manner in which specific courses are conducted, and adaptation of evaluation procedure.
B. Personally owned auxiliary aids shall be permitted in the classroom and in campus buildings (such as tape recorders, listening devices, service animals, wheel chairs, etc.). Other auxiliary aids or devices which are necessary to enable the student with a disability the fullest possible opportunity to learn or participate in activities shall be provided by the University.
(a) Reader service for the legally blind will continue to be provided by the University according to the Indiana Law and other University policy.
(b) The University is not expected to provide attendants, individually prescribed devices, readers for personal use, or other devices or services of a personal nature.
(c) Individuals with physical disabilities, with authorization, shall be allowed to purchase a parking permit for space near the academic and activity buildings.
C. Personal, academic or vocational counseling, guidance and placement services shall be given to students with disabilities in such a manner as not to deter or discourage them because of difficulties in accessibilities or the curriculum, course, or activity; and shall not be given in such a manner as to influence the individual toward more restrictive career objectives than are individuals without disabilities with similar interest and abilities.
In consideration of the foregoing, the following policy and procedures have been adopted at the Purdue University North Central Campus:
Policy: It is the policy of Purdue University North Central to assist qualified individuals with disabilities in obtaining all reasonable and appropriate accommodations which may be required due to a disability. This function is to be administered and coordinated by the Disability Services Coordinator under the direction of the Student Support Services Director and the Dean of Students in collaboration with the 504 and ADA Compliance Officer. Implementation of approved accommodations is the responsibility of faculty and staff. The responsibility for requesting appropriate and reasonable classroom and testing accommodations is that of the student. In specific cases where an individual with a disability is unable to make arrangements for accommodations, or is in need of support in doing so, the Disability Services Coordinator (hereafter Coordinator) will intercede on the individual's behalf.
The University is required, by law, to provide accommodations to those individuals who have provided the University with documentation of a disability. By statute a person with a disability “means an individual who: (A) has a physical or mental impairment that substantially limits one or more major life activities; (B) has a record of such an impairment or; (C) is regarded as having such an impairment.” Major life activities include: self-care, manual tasks, walking, seeing, hearing, speaking, breathing, sitting, standing, reaching, thinking, concentrating, reading, interacting with others, learning or working. Under current Supreme Court rulings ADA does not cover people whose conditions can be “mitigated” with medication, eyeglasses, or similar corrective measures.
1.0 Identification of Individuals with a Disability
1.01 Self Identification
Students with disabilities are identified in several different ways. The most common is self identification. The University Admissions Office sends a Disability Information Request Form to each student admitted to the University. Students are instructed to complete the form and return it to the Disability Services Coordinator.
1.02 Identification and Referral by another University Department
Most commonly this will be by an academic advisor who sees the student for an intake advising interview. However, frequently professors will identify a student in a class and refer them. To facilitate this process, it is recommended that all faculty include a statement on their course syllabi to the effect: “If you have a disability which will require testing or classroom accommodations, please see me during the first week of classes.” Faculty are also encouraged to verbally reinforce this invitation during the first week of classes.
1.03 Referrals from an outside agency
The Indiana Office of Vocational Rehabilitation has made a practice of referring all of their Purdue University North Central clients to the Coordinator. Other agencies may do same.
2.0 Response to Disability Information Request Form
2.01 Coordinator will do follow-up by phone, letter or in-person interview with any student who requests information or is referred to the office. Faculty and staff who identify students with disabilities may provide information to them on how to contact the Coordinator.
3.0 Required Disability Documentation
3.01 Medical statement from a physician attesting to an individual's physical disability, functional limitations as a result of the disability and recommended reasonable and appropriate accommodations. Forms which the student may use to solicit this information are available from the Coordinator.
3.02 Medical statement from a psychiatrist, psychologist or licensed therapist attesting to an individual's emotional disability, functional limitations as a result of the disability and recommended reasonable and appropriate accommodations. Forms are available from Coordinator which the student may use to solicit this information.
3.03 Evaluation from an appropriate professional attesting to the existence of a learning disability, functional limitations as a result of the disability and recommended reasonable and appropriate accommodations. This evaluation should include achievement and intelligence test scores. Documentation must be no more than three years old.
4.0 Accommodations Approval
4.01 Upon receipt of a request for an accommodation, faculty and staff may contact the Coordinator for verification of eligibility. Students who are participating in Student Support Services will make an appointment with the Coordinator through Student Support Services Office. All other students will be processed through the Dean of Students Office.
4.02 During the initial interview, the following topics will be covered and appropriate forms completed.
(a) disability documentation will be reviewed;
(b) functional limitations as a result of the disability will be discussed;
(c) previous accommodations and services will be reviewed;
(d) agreement will be obtained as to what accommodations will be provided; including, but not limited to physical access; classroom; and information access;
(e) clarification of roles and responsibilities: Coordinator, student, faculty; staff;
(f) the following forms will be completed: Information Release Form; Accommodations Check List and, as appropriate, Testing Accommodations Request Form, Classroom Accommodations Form or other related forms.
(g) The Coordinator will maintain a documentation file in a secure location and a working file.
5.0 Notification of Need for Accommodations
5.01 It is the responsibility of the student to notify faculty or staff of the need for accommodations so that they may fully access academic, non academic or extra curricular programs, services and activities offered by the University. Accommodations are to be arranged from the date of request. Accommodations are not retroactive.
5.02 An Information Release Form must be completed and signed by the student before the Coordinator can share information regarding the student's status. This form is to be kept on file by the Coordinator.
5.03 For underclass students requesting academic accommodations, the Coordinator will prepare a written memorandum which identifies the student, the course, the division and semester, the approved accommodations and introduces the Coordinator as the resource contact for the faculty member. The student is responsible for presenting the memorandum to the faculty member and discussing the contents of the memorandum as well as any additional concerns he/she may have in regard to any arrangements that may need to be made to implement the accommodations. Upperclass students are expected to work directly with the faculty member in making their requests and arrangements.
6.0 Testing Accommodations (including but not limited to classroom testing, placement testing, and/or assessment measurements such as career battery)
6.01 Faculty/staff may upon request make arrangements for testing accommodations directly with a student.
6.02 Upon request, the Coordinator will provide the student with a Testing Accommodation Request Form. The Coordinator will complete the information section and check the approved testing accommodations, e.g., extended time, enlarged print, distraction reduced environment, etc.
6.03 The Coordinator will give the Testing Accommodation Form to the student who is, in turn, responsible for presenting it to and discussing it with the faculty/staff member.
6.04 Faculty/staff member makes arrangements to implement the accommodation.
6.05 Assistance in implementation of testing accommodations is available through the Student Success Center or, in the case of students who will require a reader or a scribe, the Coordinator. If the faculty member chooses to request assistance in implementation of a classroom testing accommodation from either the Student Success Center or the Coordinator, the faculty member and the student will discuss and indicate on the Testing Accommodation Form whether the request is for blanket approval or a single test approval; if there are any additional testing aids permitted by the professor, e.g., use of textbook, class notes, calculator or computer; how the test will be delivered to the assisting office when it may be administered; and how it is to be returned to the professor.
6.06 The professor will sign the form, retain the yellow copy and return the remaining copy to the Coordinator in person, by campus mail or have the student do so.
6.07 The student will schedule with the Student Success Center or the Coordinator the date and time of test administration. All tests must be scheduled a minimum of 3 working days prior to administration of the test.
6.08 For those students who need a reader or scribe, the Coordinator or designee will read or scribe the test. If the accommodation is being supervised by the faculty member, the faculty member will be responsible for making these arrangements.
6.09 If a student comes to the Coordinator or Student Success Center with an unscheduled test, the test will be administered only if a Testing Accommodation Request Form is in the student's file and a staff member is available to proctor the test. If the Coordinator of Center is unable to proctor the test, the student will be instructed to: schedule a time in which the test can be properly proctored; or return the test to the instructor and request that it be proctored at a later time.
6.10 After a test has been administered, a Testing Accommodations Report Form will be completed by the person who has administered the test. A copy will be returned with the test to the professor per the instructions provided; a copy will be placed in the student's file; and if the accommodations have been provided by the Student Success Center, a third copy of the Testing Accommodations Report Form will be provided for recordkeeping purposes to the Coordinator.
6.11 Any conflicts or problems that should arise in regard to testing accommodations will be resolved by the Coordinator. If it is not resolved to the satisfaction of the student or faculty member, either may appeal to the Dean of Students and/or the 504/ADA Compliance Officer.
7.0 Classroom Accommodations
7.01 Appropriate and reasonable classroom accommodations will be determined by the Coordinator, in consultation with the student, based on the diagnosed disability.
7.02 The Coordinator will assist the student in obtaining required classroom accommodations, including, but not limited to, note takers, special seating, table accommodations, change of classroom location, etc. A Classroom Accommodation Request Form is to be completed and signed by the student and Coordinator each semester documenting the specific accommodations to be provided.
7.03 In the event that a note taker is required for a class, the student may request the instructor to seek a volunteer within the class or request assistance from the Coordinator in identification of an appropriate notetaker.
7.04 The Coordinator will, upon request of an eligible student, provide note taker binders and two ply non-carbonized note paper. Supplies are to be signed out. At the end of each semester, the note taker binders and all unused paper is to be returned to the Coordinator.
7.05 The Coordinator will make arrangements for the provision of typing service provided for eligible students if:
(a) student is unable to type due to their disability.
(b) typing services will be provided only during the transitional time needed for the student to learn how to use available adaptive equipment located on campus.
(c) it is the responsibility of the student to provide material to be typed a minimum of 3 working days prior to required deadline to order to allow for completion in a timely manner.
(d) all materials will be typed as presented. No proofing will be provided.
8.0 Print Alternatives
The Coordinator will assist individuals with documented disabilities which result in functional limitations related to reading and/or other print related impairments to obtain audio textbooks, texts scanned to CD, readers or instruction on how to use assistive technology available on campus (Computer Lab, Student Success Center , sSs, Library).
8.01 All requests for audio textbooks must be made a minimum of 6 weeks prior to the date the material is needed.
8.02 Students requesting audio textbooks will provide a copy of the required textbooks to the Coordinator.
8.03 The Coordinator will order or assist the student in ordering books from Recording for the Blind & Dyslexic (RFB&D), the Lake County Library (LCL) or other sources.
8.04 If textbooks are not available from outside agency sources, alternative arrangements may be made with the Coordinator.
8.05 Dependent upon the needs and abilities of the student requesting alternate print medium, materials may be scanned to a readable CD and the CD provided to the student. It is expected that the individual making such a request will supply one copy of the textbook to be scanned. Material scanned to disk may be accessed using voice software.
8.06 A CCTV is available in the library to enlarge print and, thus, assist with reading. Computer fonts can also be increased and then the document may be printed to provide enlarged print. The Student Success Center can provide additional assistance
8.07 Readers will be provided for testing accommodations. Readers will be provided on a limited basis dependent on needs and documentation for class related access, but are not available for personal study time or personal/recreational reading.
9.0 Physical Accommodations
9.10 The Coordinator will meet with each individual with a disability to determine if physical accommodations are needed due to a disability.
9.02 The Coordinator will contact the appropriate university personnel to request that physical accommodations be made in classroom location and/or setup.
9.03 Disability parking permits are available in the Police Department. If a student has a state disability placard or plate, they are not required to obtain a campus disability permit.
9.04 Room changes are to be discussed with the Registrar's Office.
9.05 Classroom setups are to be discussed with the Building Services Supervisor.
9.06 Campus or building access concerns or problems are to be reported to the Coordinator and/or directly to the Physical Plant Maintenance Supervisor. This would include, but not be limited to, snow removal, sidewalk repairs, automatic door and elevator malfunctions, restroom access, etc.
10.0 Adaptive Equipment
10.01 The Coordinator will respond to student requests for adaptive equipment based on established procedures for provision of accommodations.
10.02 The Coordinator will maintain a list of the current adaptive equipment on campus.
10.03 The Coordinator will coordinate instruction in the use of the adaptive equipment by students with disabilities, University staff and faculty.
10.04 The Coordinator will coordinate with Information Services recommendations for the purchase of computer related adaptive equipment or software.
10.05 The Coordinator will serve as a campus resource person in relation to adaptive equipment.
10.06 The Coordinator will recommend purchase of adaptive equipment to appropriate Deans, department chairs and/or directors.
11.0 Referral of Individuals with a Disability
11.01 The Coordinator will actively interact and network with community agencies which provide assistance to individuals with disabilities.
11.02 The Coordinator will maintain a listing of the various university departments and outside agencies available to offer assistance to an individual with a disability.
11.03 The Coordinator will intercede on behalf of an individual with a disability in accessing information from another university department or outside agency if the student is unable to do so or is in need of transitional support in obtaining information or assistance.
11.04 The Coordinator will refer individuals with disabilities to outside agencies in those cases where it is appropriate to do so and/or in cases where the university resources are insufficient.
12.0 Advisory/Educational Services
12.01 The Coordinator will develop workshops and/or presentations related to disability awareness, accommodations, and the impact of federal and state laws.
12.02 The Coordinator will annually review and update, if necessary, the Faculty Resource Guide: An Equal Opportunity – Alternatives for Teaching Student with Disabilities.
12.03 The Coordinator, in collaboration with the EO/AA Officer, will act as a resource person in regard to disability issues and accommodations for faculty staff, administration and students.
13.01 The Coordinator will, at regularly scheduled intervals, survey individuals who have used accommodations to obtain feedback to use in evaluating the disability services component.
13.02 The Coordinator will, at regularly scheduled intervals, formally evaluate the services provided.
14.0 Fiscal Responsibility
14.01 Individual academic sections and campus departments or units are responsible for purchasing usual and customary supplies, services and equipment which may be required for implementation of reasonable and appropriate accommodations within their areas of responsibility. Financial support for items which transcend “usual and customary” may be requested, in writing, from the EO/AA Officer who is also the 504/ADA Compliance Officer. Requests will be evaluated on the basis of merit, need and available fiscal resources.
15.0 Grievance Procedures
